The following excerpt is from page 14 of the PC-DOS "Getting Started - Upgrade" version 5.0 manual:

MOUSE.COM is a mouse driver for the IBM mouse. The MOUSE.COM program that came with the IBM Mouse has been upgraded to support the DOS 5.0 Shell. MOUSE.COM is not automatically installed by the setup program.

NOTE: Copy MOUSE.COM to your DOS directory if you are using an IBM mouse. If you are using any other mouse, DO NOT copy MOUSE.COM.
Thus, if you are using ANY type of Microsoft Mouse on a machine on which you are installing PC-DOS 5.0, do NOT install the MOUSE.COM driver supplied with the PC-DOS 5.0 upgrade. You should instead load your previous Microsoft Mouse driver version (that is, don't make any changes for the mouse driver if you are using a Microsoft Mouse).
