Creating a Locked Template Form with Underlined Fields

SUMMARY

The following procedures allow information to be entered into a template form in Microsoft Word for the Macintosh without modifying the template form.

When creating a template form document, the space for user-entered information should be identified by a series of symbols -- for example, ****. To have an entry underlined, select the series of symbols and format them as underlined.

To enter information into the form, use the Find command to scroll through the document:

  1. Position the insertion point at the beginning of the document.

  2. From the Utilities menu, choose Find.

  3. Enter the information field indicator, ****, in the Find What box. When the first match is found, click in the document and enter the desired information.

  4. Press COMMAND+OPTION+A after completing step 3 to find the next occurrence of an entry field.

If the document has been locked, Word will prompt you to save it with a new name when you try to close it.

After the document is saved, do the following to prevent modification to the designated form document:

  1. From the File menu, choose Quit.

  2. Select the document by clicking it once.

  3. From the File menu, choose Get Info.

  4. Select Locked in the upper-right corner of the Get Info dialog box.
